Living Well – a case study

This week we’d like to share a great video that highlights a project in Cornwall also working to reduce isolation and improve health and wellbeing.

The Living Well project aims to help people who are socially isolated and highly dependent on health and social care services to improve their quality of life. It starts with a conversation to understand the person’s story and what will make life better. It aims to reconnect people with their communities – facilitating change rather than fixing problems, building on the support infrastructure that’s already there.
